Steps to fill out an online form-

  • One must visit the Passport Seva Website first and then register by selecting the Register Now link on the home page.

  • Now, one must access the Passport Seva Online Website using the registered Login Id.

  • Then they must simply click for a New Passport/Re-issue a Passport to proceed.

  • They must fill out application Form No. II for Miscellaneous Services with the necessary data and then submit it, such as a certificate of marriage or an analogous form of documentation.

  • Afterward, they must go to the See Saved/Submitted Applications screen and select the Pay and Schedule Appointment option to make an appointment. One must keep in mind that you must now pay online to make an appointment at any Passport Seva Kendra or Passport Office.

  • One must click the Print Application Receipt link to print the application receipt with the Application Reference Number/Appointment Date.

  • One must visit the Passport Seva Kendra/Regional Passport Office, where an appointment, along with the original papers, has been scheduled after paying a payment.



Steps for Submission of e-Form

One must be aware that, for your convenience, facilities have been established to make it easier to submit an application online, just like any other kind of official document.

  • The e-form must be downloaded from the main page's download e-form link, filled out, and then saved by clicking the Validate and save button.

  • It will create an XML file that will later need to be uploaded into the system.

  • Use the registered login ID to access the Passport Seva Online Website by following the same steps as above. After successfully login in, one must select Upload e-Form to submit the XML file.

  • The system only allows XML files. Therefore you shouldn't post the PDF form at this time.

  • One must follow the same payment procedures in order to schedule an appointment at your neighbourhood Passport Seva Kendra or the Regional Passport Office using the original documents.

Documents needed for the Surname change in the Passport

  • A photocopy and the original of the marriage certificate (self-attested)

  • Old passport in original with self-attested copies of the first two and last two pages, the page of observations (if any) made by the passport issuing authority, and the page of validity extension, if any, in the case of a passport with such a short validity.

  • Evidence of current residency

  • Education documentation which includes 10th, 12th, and graduation certificate (if completed)

  • A copy of the passport or spouse's paperwork

  • From the organization in charge of the location where the certificate was issued, through Indian Mission/Post

  • Consulate, High Commission, or Embassy of that country in India
